3. Complete your details to complete your booking 
4. You’ll receive a confirmation email with the next steps once the booking has been confirmed.

The health assessments will be offered over the next two-years. Health assessments are first-come, first-served with more dates being added throughout the year. If you’d like to be added to a waiting list to hear of the next available dates, please complete this form.

For factory-based workers: if you are on shift please speak to your Shift Manager to agree a suitable time before booking.
